The Relationship between Juxtapapillary Duodenal Diverticula and Choleithiasis



Abstract
ackground : The influence of the juxtapapillary duodenal diverticulum (JPDD) on the
biliary tree and the pancreas was first described by Lemmelin,1934. Recently a positives
correlation between the JPDD and choleithiasis was reported. However, it is still uncertain
whether a definite causal rslationship exsists between the two.
In the present study, we have examined the possibility that JPDD are associated with
cholelithiasis. And the type of JPDD was examined to clarify the clinical rloe of the JPDD in
the formation od gallstone.
Method : The result of 534 ERCP examination were analysed in order to assess whether
or not-JPDD are associated with cholelithiasis. Cholelitiasis was classified to
chledocholithiasis, cholecystolithiasis and combined case. Also, incidence of cholelithiasis in
JPDD case was analysed in according to relationship between diverticular and ampulla of
Vater.
Results : JPDD was found in 153 (28.7%) patients in whom 91(59.5%) had cholelithiasis.
381 patients did not have JPDD in whom 163 (43.3%) had cholelithiasis (p<0.001).
Choledocholithiasis was closely related to JPDD, but cholecystolithiasis was not. No stastical
correlation between type of JPDD and incidence of cholelithiasis.
Conclusion : The result of this study shows significant association between JPDD and
cholelithiasis but further studies is recommended to assess to define pathogenic mechanism.
